Safeguard client information and Protected Health Information (PHI) and maintain HIPAA and applicable privacy compliance.
Follow SPRG's placement and transfer procedures and coordinate client transfers through the Organization.
We receive and review placement referrals.
We identify providers based on client needs and availability.
Relevant client details are shared with suitable providers.
We help coordinate communication, questions, and tours.
The client or authorized representative chooses the provider.
We coordinate the move-in process and provide ongoing support.
